This was accomplished with a neat trick. Trial participants agreed to be treated by Pfizer's in-house study doctors (availability 9-5?). When one has a life-threatening emergency, one goes to the emergency room of the nearest hospital. In doing so, one violates the rules and is stricken from the study.
